If he didn't choose Mizuho he would have been disgraced, kicked out of school, and disowned by his uncle. And if he dared to cheat on Mizuho, he is even worse than Takayuki or Rin... he's heading towards the Makoto zone. And besides, if Mizuho hadn't arrived on the scene he would've probably chosen Ichigo. Herikawa will have to make do with the sensei, hopefully AFTER graduation. Kei simply couldn’t choose Herikawa.
